Note:
If you have a Qualifying Life Event, you must complete the Life Event in MyUltiStuff within 30 days of the event occurring, otherwise you will need to wait until the next annual open enrollment.

Medical Plans


You can choose to enroll in one of three UnitedHealthcare (UHC) plans, or the Kaiser HMO Plan (if you live in Southern California). Each of the medical plans includes pharmacy benefits (Rx). Each UHC plan (Bronze, Silver, or Gold) has different bi-weekly premiums, deductibles, coinsurance, Out-of-Pocket-maximums, and employer HSA contributions.

UHC Bronze

Deductible:

$3,500 (I) / $7,000 (F)


Coinsurance:

40% after deductible


Generic Drugs:

20% after deductible


UHC Silver

Deductible:

$2,800 (I) / $5,600 (F)


Coinsurance:

25% after deductible


Generic Drugs:

20% after deductible


UHC Gold

Deductible:

$1,900 (I) / $3,800 (F)


Coinsurance:

20% after deductible


Generic Drugs:

20% after deductible


Kaiser

Deductible:

None


Coinsurance:

$25 doctor visit

Prescription Drug


When you enroll in a UHC or Kaiser medical plan, you are automatically covered under the prescription drug program.

Note: If you (and/or your dependents) have Medicare or will become eligible for Medicare during the benefit plan year, federal law gives you more choices about your prescription drug coverage. When you (and/or your dependents) become eligible for Medicare, information regarding Medicare Part D will automatically be sent to you by the medical plan.

Dental Plan


Dental care is important to your overall health and wellbeing, and to having a great smile! If you enroll in a Jack in the Box medical plan, you and your eligible dependents may enroll in quality dental coverage offered through Ameritas.

Coverage includes preventive & diagnostic services, orthodontia, and basic and major services. You receive the greatest savings using a provider within the Ameritas network.

Basic:

Major:

50% after deductible


Out-of-Network

Deductible:

100% of R&C, no deductible

80% of R&C, after deductible

Vision Plan


Vision care is also important to your overall health & wellbeing! If you enroll in a Jack in the Box medical plan, you and your eligible dependents may enroll in quality vision coverage through VSP (Vision Service Plan). Coverage includes exams, prescription glasses, contacts, diabetic eyecare, and other savings. You receive the greatest savings using a provider within the VSP network.

Jack in the Box Inc. ("Jack") permits INSURERS to offer its employees certain voluntary insurance programs. Whether you choose to enroll in any of these programs is completely optional and voluntary. JACK does not make a contribution towards the cost of these programs and employees pay the full cost of premiums on an after-tax basis. Jack in the Box does not sponsor, maintain, endorse, recommend, or promote these voluntary programs. JACK'sinvolvement regarding these voluntary insurance programs is strictly limited to allowing the insurer access to employees to publicize these programs and JACK may perform certain ministerial functions such as payroll deduction and forwarding employee premium payments to the insurer. JACK does not receive any consideration in the form of cash or otherwise in connection with the program, other than reasonable compensation, excluding any profit, for administrative services actually rendered in connection with payroll deductions. Accordingly, these voluntary insurance programs are not subject to ERISA and related regulations. All questions or claims regarding these programs should be directed to the insurer.

In the event of any conflict between the content contained herein and the Summary Plan Description (SPD), the provisions of the Summary Plan Description (SPD) shall prevail.
